Background, Family, and Health Form (Adolescent)

These forms have roots in treatment preadmission assessments such as the TCU Intake
(see Joe, Simpson, Greener, & Rowan-Szal, 2004).

TCU Adolescent Risk Forms (A and B versions; RSKForm) – document age, gender, race/ethnicity, education, employment, family involvement, parent’s employment and education, and living arrangements (A) and school, criminal activities, and mental health and substance abuse treatment (B).

TCU Adolescent Family, Friends, and Self Youth Form (FFSFORM) – documents family and peer functioning along six dimensions: family warmth, family control, family conflict, peer trouble, peer to family, and peer socialization (Simpson & McBride, 1992).
